The Home and Community Care program is committed to helping Elders live safely, independently, and with dignity in their own homes. Using a trauma-informed approach, we provide compassionate, culturally sensitive care through a range of services including personal care, homemaking, and referrals to appropriate health and community resources.

Our team also offers continuity of care for clients during hospital admissions. We work collaboratively with health authorities to ensure each client receives coordinated, holistic care that supports their overall well-being.

Our Services

  • Personal Care: Assistance with daily activities such as bathing, grooming, and dressing.
  • Basic Homemaking Support: Light housekeeping, laundry, and meal preparation to support independent living.
  • Referrals: Occupational Therapy, Physiotherapy, Foot Care, and Wound Care.
  • Home Safety Assessments
  • Elder Abuse and Neglect: Information and Response.
  • Hospital Support: Follow-up care for clients during and after hospital stays.
  • Palliative Care Collaboration: Integrated support for clients with advanced or terminal illnesses.

How to Become a Client

Anyone living in community may be referred to the Home and Community Care program. Referrals can be made by family members, health professionals, or the clients themselves.

To refer a potential client:

  1. Download and complete our referral form here.
  2. Email the completed form to hccnurses@squamish.net.
  3. Our team will contact the client within 24 to 48 hours to arrange a home visit for an initial assessment.

The Government of B.C., through the Ministry of Health, provides funding to the United Way British Columbia to manage the Better at Home program, which provides support to Elders aged 65 and older. This program aids Elders in carrying out basic day-to-day tasks, enabling them to preserve their independence at home and remain connected with their community. Better At Home services are not of a medical nature and instead include light housekeeping, light yard work, friendly visits, and companionship. What sets this program apart is its distinctive “seniors planning for seniors” approach, where Elders actively participate in designing, running, and evaluating the program.
